The Metropolitan Police Department suspects that the inadequate ventilation and the operator's failure to take proper safety measures at the facility, which had equipment to pump water from the source of a hot spring in an enclosed space in the basement, caused the explosion in which three people were killed and eight injured. The MPD searched 10 locations on suspicion of professional negligence resulting in death and injury Wednesday. The locations included the operator of the facility, Unimat Beauty and Spa Inc., in Minato Ward, the builder of the facility, Taisei Corp. in Shinjuku Ward, a firm that was contracted to do maintenance work in the facility's water-related areas, Hitachi Building Systems Co. in Chiyoda Ward, and the company responsible for checking the water-pumping equipment, Sangu Co. in Shinagawa Ward. According to the MPD, Shiespa was designed with special structural features, as the facility was erected in a limited space near a crowded entertainment district. In the basement of the annex, water, pumped from the source of a hot spring about 1,500 meters underground, passed through a gas separator to remove methane. The water was then stored in a tank and, after methane gas was removed again, pumped to the spa in the main building. Until the water was transferred to the main building, all processes were conducted in the highly-confined basement of the annex. In addition to pipes for methane removal that were part of the gas separator and the water tank, there was a ventilator above the water tank for expelling the gas outside. The MPD's inspection Wednesday found areas near the gas separator and the water tank had suffered the most severe damage. The MPD concluded the explosion occurred after methane filled these areas. If the ventilator had worked properly, the gas would not have filled the areas to the point of causing an explosion, according to the police. In the basement of the annex, no methane detector alarm was installed. Unimat Beauty and Spa had a contract with Hitachi Building Systems for the maintenance of the annex. Hitachi Building Systems subcontracted operations to three firms, including Sangu, which was in charge of checking the water pumping equipment in the basement. Sangu kept records of a pressure gauge and a flowmeter, but did not check the density of methane. Sangu employees have told investigators that they only conducted visual checks of the ventilation devices, as such checks were not included in the contract with Hitachi Building Systems. Checks on ventilation devices were not included in the contract between Unimat Beauty and Spa and Hitachi Building Systems. Sources said equipment to separate and release into the atmosphere methane that rises with hot water when the water is piped from 1,500 meters underground probably had not been checked since the spa opened in January 2006. Police focused their investigation Wednesday on whether the methane may have become trapped inside the Shiespa building, causing the blast. Police also questioned Shiespa's operator, Unimat Beauty and Spa Inc., to determine how the facility was managed and searched the firm's premises. The company, however, claims the annex basement was equipped with a detector, and said a security alarm at the office of the main building would have gone off if there were any problems. The company said it found no problems before the accident. But police said they have yet to find a detector at the site. They plan to question company officials about how the detector and alarm were installed and operated, they said. The explosion, which destroyed the annex building, killed Mari Hizume, 51, Hiromi Fujikawa, 22, and Akina Senzai, 23. They were all Shiespa employees, police said. Two of the eight injured — Naomi Kitamura, 35, and Kimie Ikeda, 32 — were also employees, while a third was a 28-year-old male passerby. At the annex, natural gas that rises when hot water is piped from underground was supposed to go through a gas separator and then vented outside by fan. Police suspect that some fault in the exhaust system allowed natural gas to fill the annex building. The Tokyo Metropolitan Government said it started checking the safety procedures at 144 spa facilities under its jurisdiction. However, the Tokyo Fire Department said later the boiler was not located in the annex but in the main building. It suspects that the explosion may have been caused by methane gas contained in hot water pumped from underground. The Shibuya Police Station was also looking into the cause of the explosion, suspecting possible negligence on the part of the bathhouse operator.
